Six
months of continuous employment at a PCSI job site. |

The
employee has verbally requested assistance from PCSI - manager,
trainer or Rehabilitation Specialist. |
|

|
The
"Employees Placement Assistant Form" is filled out
and returned to manager. The manager will then send the sheet
to the Rehabilitation Specialist. |

•
|
The
"Staff Placement Assistant Form" is filled out by
the supervisor or trainer and sent to Rehabilitation Specialist. |
|
•
|
Once
the Rehabilitation Specialist has both forms, a case will be
open on the employee. |
|
•
|
PCSI
will refer the employee to the appropriate agencies in the area.
Example of service providers follows:
- MHMR
- Texas Rehabilitation Commission
- The Workforce
- State of Colorado
- Veterans Affairs
- Other Private Services Agencies |

|
PCSI
will assist in placement related services. |
|
•
|
When
an employee is placed in the community with assistance for PCSI,
the Rehabilitation Specialist will provide follow-along services
for 60 days. |
|
•
|
If
the employee fails to maintain this competitive employment -
PCSI will take this person into special consideration when fill
PCSI vacancies. |
